Oklahoma Water Watch Forms
Interested in becoming a Certified Water Quality Monitor?
The Program Profile is your first step. The Profile tells
us know who you are, the water body you wish to monitor, when
you would like to begin, and why you want to participate.
Only one program profile should be completed per group by
the Group Coordinator (GC).
After sending in your program profile and hearing back from us, and
prior to testing, a pledge form must be filled out by every monitor.
This form verifies your commitment to the program and willingness to
follow all Water Watch guidelines.
The training packet takes you through all levels of training
necessary to become a Certified Monitor. This includes Phases
I, II, and III and Advanced Nutrient training. This paperwork
should be filled out by each monitor under the tutelage of
an OWW Training Coordinator or the GC.
A monitor roster must be completed for every new group
with a complete list of names and addresses. This will allow
you to receive the official newsletter of the OWW program,
the Oklahoma Water Log, and important program updates.
