The following documents were created by a joint effort
of the state and federal funding and regulatory agencies that make up the
Funding Agency Coordinating Team (FACT) to
provide water and wastewater applicants a single, uniform method for
requesting funding and regulatory approvals.
All documents below are in Adobe® Acrobat
(.PDF) format and require the Adobe®
Acrobat Reader software to view and print. You may download a free
copy of the reader software from Adobe's®
web site. If you are unable to access the documents in this format, contact
us and we will mail or fax them to you. Only clearly legible forms printed
on white paper with a laserjet or inkjet printer will be accepted.
